First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Our staff understands that your time is valuable and many straight forward treatments can be accomplished after consultation if you meet the appropriate criteria to do so.  Patients with complex medical history or patients with complex treatment plans may require an consultation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Fill out all registration forms and email back to us prior to scheduling your appointment
  • Bring your surgical referral slip and any x-rays if applicable.
  • Bring a written or digital list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation visit.  Individuals under the age of 18 will not be seen without a parent or legal guardian present, and related individuals who do not fit this exact criteria will not substitute regardless of relationship to the minor.

If you are having Intravenous Anesthesia for your appointment you must have an escort above the age of 18 with a valid drivers license.  You must completely inform them, and they must accept that they are to wait inside of the office for the duration of the procedure until you are qualified to be discharged.  They must also be prepared to VISUALLY MONITOR you for 24 hours after your procedure for your safety. You must trust them to drive you home, obtain your medications, start you on the medications and make sure you are following your postoperative instructions.
